SUPPLEMENTAL LIFE AND AD&D PURCHASE OPTIONS You can elect to purchase additional Supplemental Life and AD&D insurance amounts for yourself and your eligible dependents. To enroll your eligible dependents for Life or AD&D, you must enroll in the corresponding supplemental coverage. For example, to enroll your dependents in Life Insurance, you must elect to purchase Supplemental Life Insurance for yourself. Payroll deductions for Supplemental Life/AD&D and Dependent Life/AD&D insurance are deducted from your paycheck on an after-tax basis. At ages 70 and 75, Life/AD&D coverage amounts are reduced to 67% and 50%, respectively. The reductions apply January 1 following the year you turn 70 and/or 75.
EVIDENCE OF INSURABILITY Evidence of Insurability (EOI) is a confidential
questionnaire to document past and current health events used to verify whether you meet the definition of good health. The Hartford EOI is required if you and your eligible dependents enroll in Supplemental Life coverage amounts above the guaranteed issue amount ($250,000 for employees, $50,000 for dependent spouses); or add Supplemental Life or Supplemental Disability coverage after the first 30 days of your hire date. You will be alerted during the enrollment process to complete an online EOI questionnaire. It must be approved by The Hartford before coverage becomes effective.

The questionnaire is brief and only one EOI submission is needed for all supplemental life and disability plans. Most EOIs are approved immediately; however, some will require additional information. Once approved, you will not need to complete the EOI again unless you wish to increase your level of coverage.
