Life and Accidental Death & Dismemberment Benefits

BASIC LIFE AND AD&D COVERAGE Reliance Rewards provides you with Basic Life and Accidental Death and Dismemberment (AD&D) insurance through The Hartford at no cost to you.

All Life/AD&D, and Dependent Life/ AD&D coverage ends the day you terminate employment or retire. Portability and conversion options are available for both Basic and Supplemental Life Insurance, and are provided by The Hartford.

Basic Coverage

• Employee Paid • In increments of $50,000 • Up to the lessor of 5X your salary or • $1,000,000 (rounded down) • Guaranteed Issue* – The lesser of 5 times your annual earnings or $250,000 • Evidence of Insurability** – For supplemental life amounts in excess of the lesser of 5 times your annual earnings or $250,000 • See chart below for rates • Employee Paid • In increments of $25,000 • Up to the lessor of $250,000 or 50% of your supplemental coverage • Guaranteed Issue* – $50,000 • Evidence of Insurability** – For supplemental life amounts in excess of $50,000 • See chart below for rates
