Who’s Your Reason? Everyone has a reason to get healthier. Maybe it’s coaching your child’s sports team, hiking with your best friend, taking that adventure trip, or reaching a personal goal. Whatever – or whoever – your reason, taking steps to maintain and improve your health isn’t just for you. It’s for the people and things that matter in your life. Do you know your reason? • Complete your Wellness Questionnaire and Biometric Screening. The Biometric Screening and Wellness Questionnaire must be completed between October 1 and December 31, 2016. And don’t forget: If your spouse/domestic partner is enrolled in a Reliance Rewards medical plan, he or she is also eligible to complete a Wellness Questionnaire and a Biometric Screening at no cost to you. *

• New Wellness Coaching will soon be available. wellness @reliance will be introducing a new and improved coaching program. Registration and details will be available starting October 3 rd . * Spouses/domestic partners are not eligible to receive the 2018 wellness incentive.
