Disability Benefits
Employee Assistance Program
Disability benefits replace a portion of your income if you are disabled and cannot work.
BASIC SHORT-TERM AND LONG-TERM BENEFIT Reliance Rewards provides basic Short-Term Disability (STD) and Long-Term Disability (LTD) coverage through The Hartford at no cost to you. The basic STD and LTD benefits are 50% of pre-disability earnings. SUPPLEMENTAL STD AND LTD BENEFIT BUY-UP OPTIONS If you would like a higher level of disability benefit, you can purchase Supplemental STD for a benefit equal to 60% of pre-disability earnings, and you can purchase Supplemental LTD for a benefit equal to 65% of pre- disability earnings. Your contribution for the buy-up option of short-term or long-term disability benefits will be deducted from your paycheck on a pre-tax basis. If you become disabled, your disability benefit payments will be subject to federal and state income taxes.
LOSS OF INCOME: ARE YOU PREPARED?
EVIDENCE OF INSURABILITY (EOI) You will not be required to submit EOI, or “proof of good health,” when you purchase Supplemental STD and/or LTD during your initial eligibility period. If you are enrolling or electing coverage after your initial eligibility period, you will be required to submit EOI. The coverage amounts requiring EOI will not take effect until The Hartford approves your EOI application. Reliance Rewards provides basic income replacement benefits at no cost to you. But will you and your family be protected if you cannot work? Consider enrolling in the supplemental life, accidental death and dismemberment (AD&D) or disability insurance plans.

LEGAL/FINANCIAL SERVICES Members may receive legal advice, telephone consultations, and referrals to qualified attorneys including a 30-minute free initial consultation for each issue, discounted fees for ongoing legal services from the attorney, and online tools and resources. Typical legal and financial matters include: • Bankruptcy • Divorce • Civil issues • Criminal matters • Managing debt • Insurance/Taxes • Consumer issues estate law ADDITIONAL SERVICES INCLUDED Employees and their household members can receive assistance for a variety of concerns. Services include: • Childcare and school locating resources • Caregiver education and support • Adoption resources • Fitness, nutrition, and general health guidance (this is separate from the wellness@reliance program) • Convenience services • Financial aid, scholarship locator, and student loan program expertise
Reliance Rewards has partnered with the Anthem Employee Assistance Program (EAP) to provide confidential, short-term counseling assessment and referral services for all Reliance Rewards benefit eligible employees and their household members. This benefit is provided at no cost to you. The Anthem EAP provides confidential in-person or online assistance 24 hours a day, 7 days a week and can help resolve your personal issues quickly before they escalate into personal or workplace challenges. Household members may receive up to (5) free sessions with an appropriate type of counselor. Counselors are licensed behavioral health clinicians with experience in the fields of employee assistance, including: • Stress management and depression • Family and marital discord • Substance abuse • Financial or legal problems • Identity theft recovery • Childcare or eldercare issues • Gambling and other compulsive behaviors • Grief and bereavement

• Company Paid • Benefit equals 50% of monthly • pre-disability earnings, up to $5,000 per month • Employee Paid • Benefit equals 65% of monthly • pre- disability earnings, up to $20,000 per month
• Company-paid • Benefit equals 50% of weekly pre-disability earnings, up to $1,000 per week
